had conceived by one (conception), even by our father Isaac. For the “by one” here refers to the fact of the children being twins, not to the fact of their having the same father.

The point is that some might say that the difference between Ishmael and Isaac was that they had different mothers. So now the apostle goes on to show that Jacob and Esau had the same mother and father, were in fact twins, born of the same act of conception, and yet God distinguished between them, and not based on works, but on the basis of foreseen faith or lack thereof.
