For this is the word of promise, At this time will I come, and Sarah shall have a son.

One Bible Commentary on Romans 9: 9

The promise was that the heir would come from Sarah, and hence Ishmael who was born of Hagar was rejected, even though he was a literal fleshly son of Abraham. This shows that all fleshy descendants are not heirs.
